孤立性肠系膜上动脉夹层的诊治

The Diagnosis and Treatment of Isolated Superior Mesenteric Artery Dissection

目的:探讨孤立性肠系膜上动脉夹层的临床特点、诊断及治疗。方法:回顾性分析2007年1月-2011年1月笔者所在医院收治的6例孤立性肠系膜上动脉夹层的临床资料,对孤立性肠系膜上动脉夹层的临床特点、诊断及治疗进行分析。结果:4例首发症状为腹痛,2例无症状,6例均由腹部增强CT确诊,Ⅰ型1例,Ⅱ型5例。2例未作处理,3例行保守治疗,1例行腔内支架植入。结论:腹部增强CT对肠系膜上动脉夹层诊断具有重要价值,其治疗应根据患者具体情况选择合理的方案。

Objective ：To summarize the clinical features, diagnosis and treatment of isolated superior mesenteric artery dissection（ISMAD）. Methods ：The clinical data of 6 cases of isolated superior mesenteric artery dissection admitted in author＇ s hospital between January 2007 to January 2011 were retrospectively analyzed.Results ：4 cases had abdominal pain, 2 cases were asymptomatic.All of the cases were diagnosed by contrast- enhanced abdominal CT, one of ISMAD of 6 cases were type Ⅰ, the rest were type Ⅱ .2 cases that were asymptomatic did not give any medication, 3 cases were treated by conservative method.1 was treated by interventional stent separately.Conclusion ：Contrast-enhanced abdominal CT is the main tool for detection of ISMAD, the therapeutic strategies depend on particular circumstances of each case.
