健脾理气法治疗功能性消化不良脾虚气滞证及对核素胃排空的影响

A clinical trial on functional dyspepsia with spleen deficiency and qi stagnation treated by the decoction of invigorating spleen and regulating qi and its effect on radionuclide gastric emptying

[目的]评价健脾理气法治疗脾虚气滞证功能性消化不良的临床疗效及其对胃排空的影响。[方法]采用随机单盲阳性药对照方法,对纳入治疗的78例脾虚气滞证功能性消化不良患者随机分为中药治疗组(40例)和西药治疗组(38例),分别给予健脾理气方和多潘立酮治疗,治疗疗程均为4周。分别在治疗前、治疗后、治疗结束后1个月(随访时),对患者的总体症状积分、单项症状积分、核素胃排空指标进行评价。[结果]①总体症状疗效评价:中药治疗组治疗后有效率85.00%,明显优于西药治疗组(68.42%),2组比较差异有统计学意义(P<0.05)。②中药治疗组对上腹痛、上腹烧灼感、餐后饱胀不适、早饱感4个主要临床症状的改善优于西药治疗组(P<0.05)。③胃排空试验评价:中药治疗组治疗后第30、45、60、75、90min 5个时相的胃排空率与西药治疗组比较,均差异有统计学意义(P<0.05)。[结论]健脾理气方可改善脾虚气滞证功能性消化不良患者的临床症状,并可加快胃排空功能。

[Objective]To evaluate the clinical effects of the decoction of invigorating spleen and regulating qi in treating functional dyspepsia（FD） with spleen deficiency and qi stagnation, and its effects on radionuclide gastric emptying. [Methods]Eighty patients were divided into two groups for a randomized, single-blind, positive drug-controlled clinical study , and 78 cases final- ly into the efficacy of statistics, including 40 cases in the treatment group, 38 cases in the control group. The treatment group received the decoction of invigorating spleen and regulating qi, and the control group received domperidone. The course was 4 weeks, and the follow-up was 1 month. The scores of symptoms were recorded before the treatment and at the end of the 4th week during the treatment, at the end of the 1st month of follow-up. The scores of radionuclidegastric emptying were recorded respectivelyEResults]①The effective rate of the treatment group group was 85.00%, the difference between two groups had statistical significance（P〈0.05）. ②The treatment group in relieving upper abdominal pain, epigastric burning, postprandial fullness and discomfort, early satiety in four main clinical symptoms was better than the control group （ P〈0.05 ）③The gastric emptying test evaluation at 30,45,60,75,90min after treatment of the treatment group was better than the control group （P〈0.05）. [Conclusion]Decoction of invigor ating spleen and regulating qi can improve the clinical symptoms of patients due to functional dys pepsia with spleen deficiency and qi stagnation, and can accelerate gastric emptying function.
