斯皮仁诺+氟康唑治疗真菌性角膜炎临床观察

Clinical Observation of Sporanox Combined with Fluconazole for Treating Fungal Keratitis

目的:观察斯皮仁诺联合氟康唑治疗真菌性角膜炎(FK)的临床疗效和安全性。方法:采用历史对照回顾性研究,从某院2009～2011年接诊的FK患者中随机抽取50例作为研究组,再从2006～2008年接诊的FK患者中随机抽取50例作为对照组。研究组采用斯皮仁诺联合氟康唑,对照组采用二性霉素B,进行局部和全身治疗,对两组的临床疗效和不良反应进行比较分析。结果:根据疗效标准判定,研究组临床疗效优于对照组(Z=-2.177,P=0.029),疗程较对照组短(t=-5.031,P=0.000),不良反应较对照组少(χ2=4.000、3.840、13.636、3.093和9.890,P=0.046、0.050、0.000、0.079和0.002)。差异有统计学意义,均P<0.05。结论:斯皮仁诺联合氟康唑治疗真菌性角膜炎较二性霉素B疗效优越,安全性好,值得在临床推广应用。

Objective：To observe the clinical efficacy and safety of sporanox combined with fluconazole in treatment of fungal keratitis（FK）.Methods：A historical control and retrospective research way was used.50 patients with FK were randomly extracted from that were accepted by a hospital from 2009 to 2011.and assigned to the study group.So as 50 cases from 2006 to 2008 to the control group.The patients in the study group were treated with linnemann..combining with fluconazole while the control group with amphotericin B.The clinic efficacy and adverse effect were compared and analyzed between the both groups.Results：According to the standard of judging efficacy,the clinic efficacy in the study group was significantly better than that in the control group（Z=-2.177,P=0.029）,the course of treatment in the study group was significantly shorter than that in the control group（t=-5.031,P=0.000）,the incidences of all adverse effect were significantly lower than those in the control group（χ2=4.000,3.840,13.636,3.093 and 9.890,P=0.046,0.050,0.000,0.079 and 0.002）.Conclusion：In treatment of fungal keratiti,the efficacy of linnemann.combined with fluconazole is much better and safer than that of amphotericin B alone.It is worth to be spread and used.
