人性化护理干预在尿毒症并发左心衰患者血液透析中的应用

Humanized Nursing Intervention in Patients with Uremia Complicated and Left Heart Failure in Hemodialysis

目的:探讨人性化护理干预在尿毒症并发左心衰竭的患者中的应用效果。方法:选择我院2009年1月至2011年1月收治的尿毒症并发左心衰的行血液透析的患者60例,随机分为两组,对照组30例采取常规护理,观察组30例行人性化护理干预。对两组临床效果进行回顾性比较分析。结果:观察组30例治愈成功29例,1例因病情过重治疗无效死亡。对照组30例治愈26例,4例治疗无效死亡。两组死亡率比较差异有统计学意义(P<0.05)。观察组患者自我护理能力、技能及自我责任感、自我概念、健康知识的掌握水平高于对照组(P<0.01),护理干预后患者抑郁和焦虑评分显著下降(P<0.01),症状改善明显。结论:在尿毒症并发左心衰竭时血液透析是使患者心衰症状快速改善的有效方法,同时对水电解质平衡紊乱起到了纠正作用,加强血液透析人性化护理干预,可提高患得的生命质量,有重要的临床价值。

Objective：To explore the humanized nursing intervention in patients with uremia complicated with left heart failure.Methods：60 cases of uremia complicated with left heart failure in hemodialysis in our hospital from 2009January to2011January were randomly divided into two groups,30 cases in the control group,used the routine nursing care,observation group30 cases used humanized nursing intervention.Clinical results of two groups were retrospectively analyzed.Results：The observation group 30cases were cured successfully in 29 cases,1 cases died of overweight treatment because of illness.30 cases of the control group26 cases were cured,4 dead patients.Two groups of mortality rate had a significant difference（P〈0.05）.The observation group patients self-care ability,skills and sense of responsibility,self concept,health knowledge level were higher than that of control group（P〈0.01）,patients with depression and anxiety scores decreased significantly after the nursing intervention（P〈0.01）,symptoms improved significantly.Conclusion： hemodialysis is the effective method in uremia complicated with left heart failure patients,it can fast improve symptoms of heart failure,at the same time get water electrolyte balance disorder to correct action,strengthen hemodialysis humanized nursing intervention can improve the the quality of life,and has important clinical value.
