间苯三酚应用于第一产程的临床观察

Clinical Efficacy of Phloroglucinol on the First Stage of Labor

目的观察间苯三酚在第一产程中的临床应用疗效。方法选择正常初产妇伴宫颈水肿、痉挛者177例,其中分为:潜伏期90例,随机分为间苯三酚组45例和东莨菪碱组45例;活跃期87例,随机分为间苯三酚组45例和地西泮组42例。观察产程进展、分娩方式、产后出血、新生儿评分及不良反应等情况。结果潜伏期组和活跃期组间苯三酚组的宫口扩张速度均高于东莨菪碱组,差异有统计学意义(P<0.05)。两组阴道顺产分娩率和产后出血情况比较无统计学差异(P>0.05)。间苯三酚组新生儿轻度窒息占2.2%,地西泮组新生儿轻度窒息占11.9%,两组比较差异有统计学意义(P<0.05)。间苯三酚组产妇心率血压平稳,无不适主诉。结论间苯三酚对加速宫颈扩张、缩短产程具有明显作用,且对母儿无明显不良影响,值得临床进一步推广。

Objective To observe the clinical efficacy of phloroglucinol on the first stage of labor. Methods 177 normal primiparas with cervical edema and spasm were divided into latent phase 90 cases and active phase 87 cases. The latent phase group were divided into phloroglucinol group of 45 cases and east cirrhosa choline group of 45 cases. The active group were divided into phloroglucinol group of 45 cases and diazepam group of 42 cases. The rate of cervical dilation, mode of delivery, postpartum hemorrhage, scores of neonate and side effects were observed. Results The latent phase group and active phase group：the rate of cervical dilation in phloroglucinol group was faster than east cirrhosa choline group （P 〈 0.05 ）. There was no statistically significant difference（ P 〉 0.05 ）in vaginal natural delivery rate, postpartum hemorrhage and suf- focation rate of neonate. There was no statistically significant difference in vaginal natural delivery rate and postpartum hemorrhage between phloreglucinol group and diazepam group, but there was statistically signifi- cant difference in suffocation rate of neonate（ P 〈 0.05 ）, with the proportion of 2.2% and 11.9% respec- tively. Phloroglucinol group patients had steady heart rate and blood pressure, no uncomfortable complaints. Conclusion Phtoroglucinol can effectively improve cervical ditatation and shorten the length of first stage and it is well tolerated by both mother and newborn. It is worthwhile to furthcr spread phloroglucinol in clini- cal practice.
