高胰岛素对血管的损伤作用 被引量:4

摘要 高胰岛素血症及胰岛素抵抗是冠状动脉粥样硬化性心脏病(简称冠心病)发病的重要危险因素,动物实验与基础研究证实胰岛素可致动脉粥样硬化,已有研究证实胰岛素可诱发实验性动脉粥样硬化的发生;在超生理浓度下,胰岛素刺激培养的动脉平滑肌细胞增生和迁移;胰岛素亦可刺激血管内皮细胞下胆固醇聚集,以及低密度脂蛋白与动脉平滑肌细胞和单核巨噬细胞的结合、长期给予外源胰岛素导致脂异常与动脉壁增厚、胰岛素输注加速了输注动脉粥样硬化进程,胰岛素参与动脉粥样硬化的形成是导致冠状动脉性疾病的原因之一。高胰岛素血症是冠心病独立的危险因素,对于该临床现象目前有多种假说,其中最主要是胰岛素对内皮细胞的双相作用,生理浓度促进NO合成和释放,发挥抗动脉粥样硬化作用,高浓度时损害内皮,表现为致动脉粥样硬化作用;作者的研究表明,胰岛素可促进体外培养的人血管平滑肌细胞(vascular smooth muscle cells,VSMC)增殖和DNA合成,对高血压患者和正常人的VSMC给予胰岛素干预后,发现高血压患者VSMC细胞数明显增高,提示VSMC的异常增殖对高血压的发生有着重要意义。
