心-踝血管指数与颈动脉内膜中膜厚度在动脉硬化评价中的诊断效度比较

摘要 目的通过比较心-踝血管指数(cardio-ankle vascularindex,CAVI)与颈动脉内膜中膜复合体厚度(intima-media thickness,IMT)的诊断效度,了解其各自的优势及不足,为临床医生提供诊断依据。方法对同时进行CMVI和IMT检查的120例临床资料进行分析,比较其左右测定值的差异。对无动脉硬化危险因素和动脉硬化疾病35例、脑梗死患者45例和冠状动脉狭窄40例,分别进行CAVI和IMT特异度及敏感度比较,并比较各自ROC曲线下面积。结果心-踝血管指数和颈动脉内膜中膜厚度左右测定值将绝对值相同的正负浮动百分比的例数合并后,CAVI和IMT两侧测定值上下浮动在10%以内的频度分别为98.7%,29.2%。以IMT≤1.0 mm和CAVI≤9.0为相对正常值,在诊断的敏感度上CAVI无论在冠状动脉硬化或是在脑动脉硬化均明显高于IMT,而诊断的特异度CAVI低于IMT。结论在整体动脉硬化诊断效度的比较中,新的动脉硬化指数CAVI优于IMT。
