超声在下肢浅静脉术前常规检查深静脉瓣膜功能中的应用体会

Ultrasound assessment of deep venous valvular function prior to the operation on superficial varicose veins of the lower extremities:experience in clinical practice

目的应用超声两种检查方法对比研究下肢浅静脉术前常规深静脉瓣膜功能检查及术后复查的体会。方法收集200例(条下肢)在我院进行下肢浅静脉曲张(无明显下肢肿胀、色素沉着和慢性溃疡等症状)手术并在术前常规超声检查和术后6个月复查患侧下肢深静脉瓣膜功能的患者,通过平静呼吸和挤压小腿快速松开后两种方法来判断下肢深静脉瓣膜的功能,来评价哪种检查方法相对更简便、有效。结果两种检查方法显示术前下肢深静脉瓣膜功能均以正常者及可疑不全者为多,但存在差异性(P<0.05),术后都显示出下肢深静脉瓣膜功能得到明显改善,最后统计显示术后下肢静脉瓣膜功能两种检查方法数据结果相近,无明显差异性(P>0.05)。结论对于无明显下肢肿胀、色素沉着和慢性溃疡等症状的下肢浅静脉曲张患者在做术前常规下肢深静脉多普勒超声检查时,应用平静呼吸方法检查相对更简便、有效。

Objective To assess the deep venous valvular function prior to the operation on superficial varicose veins of the lower extremities with ultrasonographie approach at two different conditions. Methods 200 patients （lower extremi- ties） with varicose veins of lower extremity, who underwent surgery in our hospital, with no obvious lower limb swelling. pigmentation, chronic ulcers, and other symptoms, were recruited in this work. Ultrasound examinations for the deep ve- nous functions of lower limbs in these patients were routinely performed postoperatively and 6-month post-operation in all patients. The deep venous functions were assessed at two different conditions, i, e. , patients at quiet breathing or their legs subjected to squeezing and then quickly loosening respectively. Results At the two-status, most patients have been shown the normal or suspected insufficient function of deep vein valve, with significant difference （ P 〈0.05）. The deep vein valve function of lower limb was significantly improved postoperatively, Finally, statistic analysis showed that postop- erative venous valve functions Were similar at two conditions, with no significant difference （ p 〉0, 05）. Conclusion For patients with varicose veins of lower extremity without aforementioned complications, the routine preoperative examination for deep venous function by doppler ultrasound should be conducted at quiet breathing status because such a condition is ：onvenient and effectiveness
