V-Y advancement flap combined with skin graft around the pedicle to repair fingertip defects

目的总结V-Y推进皮瓣蒂部植皮修复指尖缺损的效果。方法2008年8月至2010年10月，采用末节掌侧或侧方V-Y推进皮瓣联合蒂部植皮的方法修复指尖缺损29例34指，其中9指同时行指甲延长术。结果术后皮瓣及植皮全部存活，伤口愈合良好。术后随访时间为3，17个月，皮瓣质地、色泽、皮纹正常，感觉与健指基本相同。植皮部位指纹重新建立。9指指甲延长0．2-0．4cm，指甲生长无畸形，外观明显改善。结论V-Y推进皮瓣蒂部植皮手术操作简单、创伤小、效果好，是一种修复指尖缺损的理想手术方式。

Objective To summarize the clinical results of V-Y advancement flap combined with skin graft at the pedicle in the repair of fingertip defects. Methods From August 2008 to October 2010, fingertip defects in 34 fingers of 29 cases were treated with V-Y advancement flap from volar or lateral aspect of the distal finger. Meanwhile full-thickness skin graft was used to cover the pedicle of the flap. Nail extension was also carried out in 9 of the 34 fingers. Results All the flaps and skin grafts survived completely. The wound healed well after the operation. Postoperative follow-up period ranged from 3 to 17 months. The texture, color and whorls and lines of the flaps were normal, almost equivalent to the normal fingers. The skin graft area re- established fingerprint. The length of nail extension ranged from 0.2 to 0.4 cm in the 9 Fingers. There was no abnormal nail growth and the appearance of the nails was significantly improved. Condusion V-Y advancement flap combined with skin graft at the pediele has the advantages of being technically simple, minimally invasive and effective. It is an ideal surgery for repairing fingertip defects.
