定点标识弧形延长法截骨矫正下颌角肥大

Localizable identifying radical extension for mandible angle osteotomy

目的探讨经口内入路定点标识弧形延长法下颌角截骨整形术的疗效。方法术前在颌骨全景X线片上定点标识，并设计弧形延长的截骨线，术中将颌骨全景X线片作为重要参照，经口内入路对下颌角进行延长的弧形截骨术。自2009年3月至2011年11月。共收治186例患者。结果术后所有患者面形均得到明显改善，下颌缘过渡自然、线条流畅。术后随访3-12个月，其中，20例患者下颌麻木，12例患者口周皮肤黏膜擦伤，均经处理后痊愈。所有患者无严重并发症发生，效果满意。结论经口入路定点标识弧形延长法下颌角截骨整形术具有基点、基线明确，易于设计，术中参照性强，术后效果良好的优点，是矫正下颌角肥大的一种较理想的手术方案。

Objective To explore the effect of localizable identifying osteotomy with radical extension for the treatment of prominent mandible angle. Methods Localizable identifying points and operative design of the radical extension osteotomy line were performed on the panoramic X-ray image of the jaw. Then according to the consultant, operations via intraoral approach were performed on 186 patients. Results All facial contour had been improved obviously. The mandible edged had natural transition and graceful lines. After 3 to 12 months follow-up, all cases received satisfactory outcome without severe complications, except for 20 cases with numb jaws and 12 cases with mouth skin mucosa grazed, which were all cured after treatment. Conclusion Localizable identifying radical extension osteotomy via intraoral approach for the treatment of prominent mandible angle has such advantages as explicit base points and baselines, easy to design, practical reference during the opera- tion, and optimal postoperative outcome. It is an ideal operation for the treatment of prominent mandible angle.
