肝脏局灶结节性增生的诊断与治疗

Diagnosis and Treatment for Focal Nodular Hyperplasia of the Liver

目的:探讨肝脏局灶结节性增生的诊断和治疗方法。方法:回顾性分析经手术和病理学检查证实的11例肝脏局灶结节性增生患者的临床资料。结果:本组73%为45岁以下患者,绝大部分无临床症状。10例为单发病壮,1例为多发病灶;肿瘤直径2.0～13.4cm,肿瘤标记物AFP、CEA、CA19-9均正常,术前MRI检查诊断准确率为75%(6/8)。均经手术切除病灶,随访4～78个月未见肿瘤复发。结论:MRI对肝脏局灶结节性增生具有较高的诊断价值,对有临床症状或诊断不明确的病例应行手术探查,手术切除是首选治疗方法。

Objective To investigate the diagnosis and treatment for focal nodular hyperplasia （FNH） of the liver. Methods Clinical data of 11 FNH patients confirmed by operation and histological examination were analyzed retrospectively. Results 73% （8/11）of patients with FNH were under 45 years of age. 82%（9/11） of patients were asymptomatic. Single tumor was found in 10 cases,two tumors was found inlease.The diameters of tumor ranged 2.0-13.4era.Tumor makers including AFP and CEA and CA19-9 were within normal range. MRI is helpful to diagnosis for FNH, correct preoperative diagnosis rate was 75%（6/8）. All patients underwent resec- tion of tumor, and there was not mortality and severe complication. No recurrence was observed after follow-up of 4-78 months. Conclusion MRI are valuable for the diagnosis of focal nodular hyperplasia of liver. Opera- tion is the optimal treatment for FNH. Surgical resection is necessary when the patient becomes sympotomatic or when there is a suspicion of malignancy.
