基于光电脉搏色素浓度谱分析的肝储备功能检测方法 被引量:6

摘要 为了克服肝储备功能检测中定时采血存在的有创伤和非连续测量问题,提出利用光电脉搏波测量方法实现色素稀释和排泄速度的无创连续测量。将吲哚氰绿(ICG)色素经静脉注入到血液中,在与血浆蛋白迅速结合后,被肝细胞吸收并排泄至胆汁中,血液中的色素浓度呈指数衰减的规律变化。选定ICG色素的光谱吸收峰805nm波长和吸光系数为零的940nm波长作为吸光度测量点,以血常规测得的血红蛋白浓度值作为参考,连续记录色素浓度稀释的第1循环曲线和浓度衰减的第2循环曲线。通过计算得到血浆消失率K和15min滞留率R15。与"金标准"定时采血法相比较,两项肝血流参数最大相对误差分别为8.12%和7.37%。本文方法满足临床要求,能有效提高肝储备功能的评估水平。 A photoelectric method is presented as a way of detecting hepatic reserving function based on the dye dilution and remove functions.For blood sampling by spectrophotometry,the measurement is invasive and intermittent.When injected intravenously,the indocyanine green(ICG) dye is rapidly and completely bound by plasma protein,removed by the liver,as well as excreted by the bile.During the period,arterial blood ICG concentration decays exponentially.Two wavelengths of the peak absorption at 805 nm and the negligible absorption at 940 nm for ICG are selected to continuously record the first circulation and the recirculation curves according to the previously measured blood hemoglobin concentration,and then ICG plasma disappearance rate(K value) and 15-min retention rate(R15 value) can be computed.Compared with the "gold standard" method of blood sampling,the maximum relative errors of the two parameters are 8.12% and 7.37%,respectively,which can meet the clinical requirement and improve the evaluation level of hepatic reserving function.
