经侧裂-岛叶入路显微手术治疗高血压基底节脑出血的临床疗效(附38例报告) 被引量:6

摘要 目的:探讨经外侧裂-岛叶入路显微手术治疗高血压基底节区脑出血的手术方法和疗效。方法:对2009-10~2010-10间我科收治的38例高血压基底节脑出血采取经外侧裂-岛叶入路清除血肿的临床资料进行回顾性分析。结果:术后1 d复查头部CT,血肿清除≥90%26例,70%~89%10例,≥60%2例。术后随访6个月根据GOS评分,恢复良好24例(63.1%),中残8例(21.0%),重残4例(10.5%),植物生存1例(2.6%),死亡1例(2.6%)。结论:经外侧裂-岛叶入路显微手术治疗基底节区脑出血是一种损伤小、疗效好、预后理想的微创手术方法。 Objective :To study the therapeutic effect for cerebral hemorrhage in basal ganglia by microsurgery trans sylvian fissure - insular approach. Methods: Retrospective analysis was performed in 38 cases with cerebral hematomas in basal ganglia region which were treated by microsurgery trans sylvian fissure - insular approach in our department from 2009. 10 to 2010. 10. Results:With 6 months follow - up study after surgery and according to Glasgow Outcome Score (GOS) ,the cases of good recovery(GR) , moderate disability(MD) , severe disability( SD), persistent vegetative state (PVS) and dead (D) were 24 cases ( 63.1% ) , 8 cases ( 21.0% ) , 4 cases ( 10. 5 % ) , 1 cases ( 2. 6% ) and 1 cases (2. 6% ) respectively. Conclusions:Microsurgical treatment trans sylvian fissure - insular approach for cerebral hematomas in basal ganglia region is a kind of minimally invasive method with less damage, better effects andmore favorable prognosxs.
