腹腔镜下输卵管节段切除术与腹腔镜下输卵管开窗术的综合疗效比较

Comparison of the comprehensive therapeutic effects of laparoscopic oviduct segmental resection and laparoscopic oviduct fenestration

目的比较腹腔镜下输卵管节段切除术与腹腔镜下输卵管开窗术的综合疗效。方法选取我院2010年9月~2011年8月的100例输卵管壶腹部妊娠患者,随机分为A组和B组,每组各50例。A组进行腹腔镜下输卵管节段切除术联合端端吻合术进行治疗,B组则进行腹腔镜下输卵管开窗术进行治疗。将两组患者的手术时间、术中出血量、排气时间、并发症发生率、术后住院时间、输卵管通畅情况、宫内妊娠率及其对卵巢的影响进行对比。结果 A组在手术时间、术中出血量、排气时间、并发症发生率、术后住院时间、输卵管通畅情况及宫内妊娠率方面均优于B组,差异均有统计学意义（P〈0.05）,而两组对患者的卵巢功能的影响差异无统计学意义（P〉0.05）。结论腹腔镜下输卵管节段切除术联合端端吻合术的临床疗效佳,且可有效保留输卵管功能,同时对卵巢影响也较小。

Objective To compare the comprehensive therapeutic effect of laparoscopic oviduct segmental resection and laparoscopic oviduct fenestration.Methods 100 patients with ampulla fallopian tube pregnancy from September 2010 to August 2011 were randomly divided into group A and group B,and each had 50 cases.The group A were treated with laparoscopic oviduct segmental resection and end-to-end anastomosis,the group B were treated with laparoscopic oviduct fenestration.The operation time,operative bleeding,exhaust time,incidence of complications,postoperative hospital stay,situation of tubal patency,intrauterine pregnancy rate and its influence for ovarian of the two groups were compared.Reults The operation time,operative bleeding,exhaust time,incidence of complications,postoperative hospital stay,situation of tubal patency,intrauterine pregnancy rate of group A were significant better than those of the group B,the differences were statistically significant（P 0.05）,and the influence for ovarian function of two groups had no significant differences（P 0.05）.Conclusion The clinical effect of laparoscopic oviduct segmental resection and end-to-end anastomosis is superior than the laparoscopic oviduct fenestration.This treatment not only preserve the fallopian tube functionality,but also minimize the influence to ovarian.
