左卡尼汀对肺炎合并心衰的疗效观察

Therapeutic effect of L-carnitine on pneumonia complicated with heart failure

目的研究左卡尼汀对肺炎合并心衰患者的疗效观察。方法选择2011年1月~2012年1月于我科住院的肺炎合并心衰的患者86例,随机分为观察组45例,对照组41例,两组均给予抗感染及常规抗心衰治疗,观察组加用左卡尼汀4 g/d,观察两组治疗前后的心率（HR）、呼吸（RR）、血氧饱和度（SO2）、N末端B型利钠肽（BNP）及超声心动图的变化。结果治疗后观察组在心率、呼吸、BNP、左室舒张末径（LVEDD）、左室收缩末径（LVESD）较对照组下降明显（P〈0.05）,血氧饱和度、左室射血分数（LVEF）增加（P〈0.05）,观察组的总有效率93.3%高于对照组的80.5%（P〈0.05）。结论左卡尼汀辅助治疗可以改善肺炎合并心衰患者的心功能,不良反应少。

Objectives To observe therapeutic effect of pneumonia with heart failure treated with L-carnitine.Methods From January 2011 to January 2012,86 patients with pneumonia complicated with heart failure were divided into two groups,the observation group had 45 patients and the control group had 41 patients.Both groups were given routine treatment,include infection control and anti-HF the raphy.The observation group treated with L-carnitine 4 g/d.The degree of clinic symptom,include heart rate,respiratory rate,oxygen saturation,BNP and the changes of ultrasound cardiogram of two groups were observed before and after the treatment.Results The two groups had significant differences on HR,RR,SO2,BNP,LVESD,LVEDD,LVEF after the treatment（P 0.05）.The total effective rate（93.3%） of the observation group was higher than that of the control group（80.5%）（P 0.05）.Conclusion L-carnitine can be combined for treating pneumonia complicated with heart failure,is worth spreading as a safe and efficient method.
