60例颅内蛛网膜囊肿显微切除报道

Microsurgical resection of 60 cases of intracranial arachnoid cysts

目的探讨蛛网膜囊肿手术指征和外科切除蛛网膜囊肿的手术效果。方法回顾分析了我院收治60例颅内蛛网膜囊肿患者,25例患者行囊肿部分切除+脑池造瘘术,35例患者行囊肿全切或近全切。结果 42例患者得到随访:22例术前症状完全消失,14例患者术前症状明显缓解,6例患者术前症状无改善,有效率为85.7%。33例患者获得影像学随访,囊肿消失26例,5例囊肿明显缩小,2例未见缩小,囊肿切除有效率为94.0%。无死亡病例。结论显微切除蛛网膜囊肿是治疗颅内蛛网膜囊肿安全、有效的方法。

Objective To explore the surgical indication and excision effect of arachnoid cyst. Methods A retrospective analysis had been made by 60 patients with arachnoid cyst. The cysts were totally removed in 35 cases, partially removed and communicated with cisterns in 25 cases. Results 42 patients were followed up, 22 of 42 cases preoperative symptoms disappeared completely, 14 cases of patients with preoperative symptoms relieved obviously, 6 cases of patients with preop- erative symptoms did not improve. The total clinical effective rate was 85.7%. 33 of 42 cases were followed up with image data. The cysts disappeared in 26 cases, decreased apparently in 5 cases and had no chance in 2 cases. The efficiency of cysts excision was 94.0%. No one died. Conclusion The excision of arachnoid cysts is safe and effective.
