前列腺外周带短T2信号结节的磁共振诊断及病理基础

MRI diagnosis and its pathological basis of hyposensitive T 2 signals in prostatic peripheral zone nodules

摘要 目的回顾性分析前列腺外周带T2WI低信号结节的影像学资料,评估MRI检查对其良恶性质的鉴别诊断价值。方法65例患者于病理检查前均进行了3.0T腹部MRI平扫及增强扫描。其中外周带低信号结节共91个,对照病理结果,癌灶结节(PCa)34个(PCa组),非癌灶结节57个(NCa组)。应用正态齐性检验、Wilcoxon秩和检验描述低信号结节的统计学意义。结果PCa组T2WI多表现为小类圆形低信号,NCa组则多数表现为条片状低信号灶。PCa组和NCa组的强化模式差异具有统计学意义(P〈0.001)。结论MRI检查有助于提高外周带T2WI低信号结节的良恶性鉴别。 Objective To evaluate the significance of MR imaging(MRI) in differential diagnosis of prostatic cancerous nodules(PCa) and non-cancerours nodules(NCa), both of which showed hypointensitive T 2 signals in the prostatic peripheral zones (PZs) on the basis of pathological changes. Methods Sixty-five cases with 34 PCa and 57 NCa underwent conventional MRI diagnosis. All these cases were included in the analysis at 3.0 Tesla before the pathological examination. All carcinoma and non-carcinoma foci on T2 signals were localized on the basis of histopathologic correlation. The data-normality test and Wilcoxon rank sum test were performed for differentiation of PCa and NCa. Results Round nodules were found in Group PCa while NCa streaky on T 2 signals mostly. The enhanced models between PCa and NCa had significant difference (P=0.000). Conclusion MRI is a useful technique in differential diagnosis for the benign and malignant hypointensive nodules on T 2 signals of the PZs.
