骨折延期愈合患者血清细胞因子水平变化规律研究 被引量:5

摘要 目的:探讨血清中相关细胞因子在骨折以及合并颅脑外伤后骨折延期愈合中的变化规律,以及神经生长因子(NGF)、血小板衍生生长因子(PDGF)与骨折愈合的相关性。方法2009年1月至2011年1月收治80例骨折患者,按有无合并颅脑外伤分为两组,单纯性骨折患者45例(单纯骨折组),骨折合并颅脑外伤患者35例(脑外伤骨折组)。检测比较两组患者的血清NGF、PDGF、白细胞介素(IL)-1、IL-6、肿瘤坏死因子(TNF)-a、C反应蛋白(CRP)、降钙素原(PCT)以及碱性磷酸酶(AKP)的含量。结果各血清细胞因子水平随着治疗时间出现先上升后下降的趋势。两组正常愈合患者术后1、3、10d血清NGF、PDGF水平均较延期愈合患者高,差异有统计学意义(P<0.05);术后3d单纯骨折组正常愈合和延期愈合患者血清NGF、PDGF水平[(86.34±10.34)、(76.344±8.97)ng/L和(267.87±83.41)、(231.78±39.45)ng/L]均较脑外伤骨折组延期愈合和正常愈合患者[(109.23±13.43)、(96.32±15.31)ng/L和(387.45±109.87)、(301.23±101.98)ng/L]低,差异有统计学意义(P<0.05)。结论NGF、PDGF等相关细胞因子水平在骨折的愈合过程中呈现一定的变化规律,对于了解患者的骨折愈合情况有很好的借鉴作用。
