改良切注术与外剥内扎术治疗混合痔的疗效比较

Modified External Resection and Internal Injection versus External Dissection and Internal Ligation in Treating Mixed Hemorrhoids

为比较改良切注术与外剥内扎术治疗混合痔的临床疗效，将77例混合痔患者随机分为两组。治疗组40例采用改良切注术治疗，对照组37例采用外剥内扎术治疗。比较两组术中出血量、主要术后并发症、创面愈合时问等指标。结果显示，两组治愈率均为100％。两组在术中出血量、创面愈合时间方面有显著性差异（P〈0．05）；在术后出血、肛门疼痛、尿潴留、肛门狭窄和肛门失禁等并发症方面差异无统计学意义（P〉0．05）。结果表明，改良切注术治疗混合痔较外剥内扎术具有术中出血量少、创面愈合时间短等优点，是一种较为理想的手术方式。

This study was to compare the clinical efficacy of modified external resection and internal injection in the treatment of mixed hemorrhoids. Seventy-seven patients with mixed hemorrhoids were random ly divided into two groups： treatment group （40 cases, receiving modified external resection and internal injection） and control group （37 cases, receiving external dissection and internal ligation）. Both groups were compared for intraopertive bleeding volume, postoperative complications and wound surface healing time. The results showed that the curative rate was 100% in both groups; significant difference was found in intraopertive bleeding volume and wound surface healing time between two groups（ P〈0.05） ; but no statistical difference was found in postoperative bleeding, anal pain, urinary retention, anal stenosis and anal incontinence,etc. （ P 〉0.05）. It is concluded that modified external resection and internal injection is an ideal surgical approach with the merits of less bleeding volume and shorter wound surface healing time.
