伽马3型髓内钉与股骨近端锁定加压钢板治疗股骨转子间骨折的疗效比较 被引量:4

摘要 目的 比较伽马3型髓内钉(Gamma 3)和股骨近端锁定加压钢板(proximal femoral locking compress plate,PF-LCP)治疗股骨转子间骨折的临床疗效.方法 对2010年7月至2011年7月收治且获得随访的53例股骨转子间骨折患者资料进行回顾性分析.根据内固定方式不同分为两组,Gamma3组28例,PF-LCP组25例.比较两组患者的手术时间、术中出血量、骨折复位质量、住院时间、术后负重时间,骨折愈合时间、术后并发症发生情况及术后6个月髋关节Harris评分.结果 53例患者术后获得9.5个月(6~18个月)随访.两组手术时间、术中出血量、骨折复位质量、术后并发症等差异均有统计学意义(均P<0.05).结论 Gamma3是股骨转子间骨折较好的内固定物之一,相对PF-LCP具有更好的优越性,但仍存在发生一些并发症的可能. Objective To compare the effectiveness of the Gamma 3 nail and the proximal femoral locking compress plate (PF-LCP) in treatment of intertrochanteric fractures. Methods From July 2010 to July 2011, 53 patients with femoral per- itrochanteric fractures were treated with Gamma 3(28 cases) or PF-LCP(25cases). A retrospective study was performed to com- pare the outcomes of the two methods regarding operation time, peri-operative blood loss, reduction, time of weight bearing, bone union, Harris score and complications. Results The patients were followed up for 6 to 18 months with an average of 9.5months. There were significant differences in operation time, perioperative blood loss, reduction, and complications between two groups (P〈0.05). Conclusion Gamma3 nail may be the first choice for treatment of intertrochanteric fractures, especially when PF-LCP management is difficult.
