子宫动脉栓塞术与病灶局部注药术治疗剖宫产术后瘢痕处妊娠80例分析

Clinical analysis of 80 cases of ectopic pregnancy in cesarean scar treated by uterine artery embolization and lesion drug injection

目的探讨子宫动脉栓塞术与病灶局部注药术治疗剖宫产术后瘢痕处妊娠的效果。方法 2006年1月至2011年8月于泰安市中心医院妇科阴道彩色B超(TVB)诊断的子宫瘢痕处妊娠80例,按住院先后随机分为两组,子宫动脉栓塞术+清宫术组(简称栓塞组)40例,阴道B超引导下病灶局部穿刺注药+清宫术(简称注药组)40例,对临床资料进行回顾性分析。结果栓塞组和注药组治愈率分别为95%和75%(P<0.05)。清宫术中出血量,栓塞组平均(50.70±4.53)ml,局部注药组(60.30±4.21)ml,栓塞组少于注药组(P<0.05)。结论两种方法均能成功保留子宫,对阴道彩超提示,病灶位于子宫肌层,血流丰富病例,选用子宫动脉栓塞术+清宫术,更安全有效。

Objective： To study the clinical outcome of cesarean scar pregnancy treated by uterine artery embolization and lesion drug injection.Methods： The clinical data of 80 patients with cesarean scar pregnancy in the Central Hospital of Taian from January 2006 to August 2011 were analyzed retrospectively in the two groups： 40 patients treated by uterine artery embolization and uterine curettage and the other 40 patients received lesion drug injection by transvaginal ultrasound-guided Technique and uterine curettage.Results： The cure rate of the embolization group（95%） was higher than that of the drug group（75%）,（P〈0.05）;the mean blood loss during operation of the embolization group [（50.70±4.53）ml]was less than that of the drug group[（60.30±4.21）ml],（P〈0.05）.Conclusion： Uterine artery embolization and lesion drug injection alsopreserve uterus.The terine artery embolization group have higher cure rate and less blood loss.The terine artery embolization are effective and safe for patients whose lesions are located in myometrium and of rich blood flow by transvaginal sonography.
