螺旋CT动态增强扫描对孤立性肺结节的诊断价值

The Value of Dynamic Contrast-enhanced Spiral CT for the Diagnosis of Solitary Pulmonary Nodules

目的探讨螺旋CT动态增强扫描对孤立性肺结节的诊断价值。方法回顾性分析经手术病理检查和临床随访证实的58例孤立性肺结节患者的螺旋CT动态增强扫描资料。结果良、恶性肺结节的平扫CT值和各期强化值、强化峰值比较无统计学差异。恶性结节以不均匀强化为主,良性结节表现多样。不均匀强化的良性结节内高、低密度分界清晰,而恶性结节呈混杂分布。点、条状强化现象仅见于恶性结节。结论螺旋CT动态增强扫描有助于孤立性肺结节的定性诊断,但应侧重于强化模式及形态变化,强化值、强化峰值的诊断价值有限。

Objective To investigate diagnosis value of dynamic contrast - enhanced spiral CT scan on solitary pulmona- ry nodules（SPNs）. Methods Retrospective analysis of 58 cases were confirmed through pathology and clinical follow-up of soli- tary pulmonary nodules with dynamic contrast - enhanced spiral CT scan data. Results There was no statistically significant difference between Benign and malignant nodules in CT value , enhancement value and strengthen distribution of peak. On the en- hanced mode, mainly malignant nodules with non-tiniform reinforcement, and variety of benign nodules. High and low density in benign nodule with non-tmiform reinforcement boundary clear, but malignant nodule appear as mixed distributions. In addition, point and/or bar abnormal enhanced signs was only found from malignant nodules. Conclusion Dynamic contrast-enhanced spi- ral CT scan can be helpful in the differential diagnosis of SPNs, but should focus on strengthening the patterns and morphological changes. The value of enhancement value and strengthen distribution of peak is limited.
