早期应用瑞舒伐他汀对急性冠脉综合征患者同型半胱氨酸和细胞间黏附分子-1的影响 被引量:4

摘要 目的探讨早期应用瑞舒伐他汀对急性冠脉综合征患者同型半胱氨酸(Hey)和细胞间黏附分子-1(ICAM-1)的影响。方法将78例冠心病患者分为急性心肌梗死组(AMI组)、不稳定型心绞痛组(UAP组)和稳定型心绞痛组(SAP组)。瑞舒伐他汀治疗前和治疗后4周采用全自动免疫分析仪检测同型半胱氨酸含量,双抗体夹心ABC—ELISA法检测血清ICAM-1浓度,并将同组治疗前后以及组间同型半胱氨酸、ICAM-1的变化进行比较。结果(1)治疗后3组患者的胆固醇(TC)、甘油三酯(TG)和低密度脂蛋白(LDL)水平均显著下降;UAP组和SAP组的HDL水平显著升高。治疗后UAP组和SAP组的胆固醇、甘油三酯、高密度脂蛋白(HDL)和低密度脂蛋白水平比较差异无统计学意义(P〉0.05)。与UAP组比较,急性心肌梗死组患者的胆固醇水平更高,而甘油三酯、低密度脂蛋白和高密度脂蛋白水平比较差异无统计学意义(P〉0.05);(2)治疗前UAP组患者血清同型半胱氨酸和ICAM-1水平高于SAP组,而急性心肌梗死组的血清同型半胱氨酸和ICAM-1水平高于UAP组。经瑞舒伐他汀治疗后3组患者血清同型半胱氨酸、ICAM-1水平均显著下降。与SAP组比较,UAP组患者治疗后血清同型半胱氨酸、ICAM-1水平均较高,而急性心肌梗死组治疗后ICAM-1水平最高。结论血清同型半胱氨酸和ICAM-1水平升高与急性冠脉综合征密切相关。瑞舒伐他汀等他汀类药物可降低急性冠脉综合征患者的血清同型半胱氨酸和ICAM-1水平,发挥其抗动脉粥样硬化和内皮保护作用。
