摘要 目的探讨Villin蛋白在贲门肠化中的作用及其与贲门腺癌(GCA)发生、发展的关系。方法采用免疫组织化学S-P法检测128例胃镜活检及手术切除的GCA及癌旁肠化组织中Villin蛋白的表达,其中贲门肠化组织25例、肠化伴不典型增生组织48例,GCA组织55例,并选取15例胃镜普查正常贲门组织作为对照。结果 Villin蛋白在正常贲门组织中无阳性表达,在贲门肠化组织中的阳性表达率为76.0%(19/25),肠化伴不典型增生组织中的阳性表达率为60.4%(29/48),GCA组织中的阳性表达率为36.4%(20/55),比较差异有统计学意义(P<0.05)。Villin蛋白的表达与GCA的分化程度有关(P<0.05)。结论Villin蛋白参与了贲门组织早期肠化和GCA的发生,并在GCA的细胞分化中起重要作用。 Objective To investigate the effect of Villin protein expression in the intestinal metaplasia at gastric cardia, and to explore the relationship between Villin protein expression and carcinogenesis and improvement of gas- tric cardia adenocarcinoma (GCA). Methods Immunohistochemical S-P method was used to detect the expression of Villin protein in the 128 gastric cardia biopsy or resection specimens including the 25 cases of intestinal metapla- sia at gastric cardia specimens, the 48 cases of intestinal metaplasia at gastric cardia with atypical hyperplasia spec- imens and the 55 cases of GCA specimens, and the 15 cases of normal gastric cardia biopsy specimens were also collected as control. Results No Villin protein expression was observed in the normal gastric mucosa, the positive rate was 76.0% ( 19/25 ) of intestinal metaplasia at gastric cardia, was 60.4% (29/48) of intestinal metaplasia at gastric cardia with atypical hyperplasia, and was 36.4% (20/55) of GCA (P 〈 0.05). The expression of Villin protein was related with the differentiated degree of GCA (P 〈 0.05 ). Conclusion Villin protein was involved in the early-stage intestinal metaplasia at gastric eardia and the carcinogenesis of GCA, and the expression of Villin protein may play an important role in the development of GCA.
