摘要
目的调查上海市区中老年骨质疏松症患者的骨折风险。方法对纳入的300例病例采用FRAXTM10年骨折风险评估表,调查患者年龄、性别、身高、体质量、脆性骨折史、父母脆性骨折史、口服激素史、类风湿关节炎史、吸烟史、饮酒史,并测定L1-L4和髋部骨密度;分析不同年龄段主要部位脆性骨折风险、髋部脆性骨折风险以及骨折风险与年龄、体质量指数、骨密度的相关性。结果①300例调查者中男女比例为1∶3.6;女性患者腰椎和髋部骨密度均明显低于男性患者(P<0.05),主要部位脆性骨折风险和髋部脆性骨折风险均明显大于男性患者(P<0.05)。②不同年龄段女性主要部位脆性骨折风险和髋部脆性骨折风险不同,主要部位脆性骨折风险以60~69岁年龄段最高(P<0.05),髋部脆性骨折风险以70~79岁年龄段最高(P<0.05)。③主要部位脆性骨折风险与腰椎及髋部骨密度均呈线性负相关(r=-0.443,r=-0.545),与男性患者体质量指数和年龄呈线性负相关(r=-0.349,r=-0.617)。④髋部脆性骨折风险与腰椎骨密度、髋部骨密度及体质量指数呈线性负相关(r=-0.405,r=0.676,r=-0.19),与女性患者年龄呈线性正相关(r=0.213)。结论上海市区中老年骨质疏松症患者中女性骨折风险较高,且主要部位脆性骨折风险以年龄在60-69岁者最高,髋部骨折风险以70-79岁者最高;骨折风险与骨密度成负相关。
Objective To investigate the fracture risk in Shanghai senior palients with osteoporosis. Methods Three hundred eases were investigated by evaluation sheet im:hnting: the age, gender, height, weight, fragility fiaeture history, parents fragility fi'acture history, hormone history, rheumatoid arthritis history, smuking history and drinking history. The bone density of LI-IA was observed ; the fracture risk at different ages, the fraeture risk of hip and correlation of fracture risk, age, weigbt index and bune density were analysed. Results ① Male and fetnale ratio was 1:3.6 in 300 eases. The lumbar vertebrae density and hip density of frmale were lower than the male ( P 〈 0.05 ), the fraeture risk at main parts and fraeture risk of hip in female were higher than male ( P 〈 0.05 ). ②The fracture risk at main paris and fraeture risk of hip in female at dift〉rent ages were different, the fraeture risk at main parts was the highest at 60- 69 years uld ( P 〈 0.05 ) , fracture risk of hip was the highest at 70 - 79 years.' oht ( P 〈 0.05).③ There was negative correlation between fracture risk at main parts aml fraeture risk of hip ( r = - 0. 443, r - 0. 545 ) , and fracture risk at main parts was negative correlated with weight index and age in male ( r = - 0. 349, r = - 0.617 ). (.4) The frae- lure risk of hip was negative eorrelated with lumbar veriebrae density, hip densily and weight index ( r = - 0. 405, r = 0. 676,r = - 0.19 ), and it was negaltive cor- related with age of female ( r = 0. 213 ). Conclusion The fi'aeture risk in Shanghai female patients with osteoporosis was higher. The main parts of fracture risk is highest at the age of 60 - 69, and the fracture risk of hip is highest at the age of 70 -79. There is negative correlation between fracture risk and hone density.
