非血栓性髂静脉受压综合征在下肢慢性静脉功能不全中的诊治意义

The significance of nonthrombotic iliac vein compression syndrome in chronic venous insufficiency

目的探讨非血栓性髂静脉受压综合征（NIVCS）在下肢慢性静脉功能不全中的诊治意义。方法对210例下肢慢性静脉功能不全（CVI）伴NIVCS患者的治疗成功率、疗效及并发症进行分析。结果本组男116例，女94例，左下肢192例，右下肢18例，平均年龄55．7岁。介入球囊扩张＋支架植入或联合浅静脉手术共210例，技术成功率100．0％。浅静脉曲张治愈率95．8％，下肢肿胀缓解率76．9％，疼痛缓解率80．2％，溃疡愈合率75．0％。平均随访32．4个月，支架总通畅率100．0％。结论纠正NIVCS或联合抗返流手术能明显改善CVI患者的症状，提出NIVCS可能是部分CVI的基础疾病。

Objective Evaluation of nonthrombotic iliae vein compression syndrome （NIVCS） pathological role in chronic venous insufficiency. Methods A retrospective analysis of patients who were diagnosed with chronic lower extremity venous insufficiency （CVI） and NIVCS was performed. Demographics, clinical presentation, management, technical success, patency rate, recurrence rate, complications were recorded in their follow-up. Results There were 210 patients with CVI and NIVCS in this study （male 116 and female 94, 192 in the left lower extremity and 18 in the right, mean age 55.7）. All patients were treated with angioplasty and stent implantation or combined with superficial vein ablation. Technical success rate was 100. 0% with the mean follow-up of 32.4 months. The relief rates of varicose veins, limb edema, leg pain and ulcerations were 95.8% , 76. 9%, 80. 2%, and 75.0% respectively during following up. Conclusion To correct NIVCS or in combination with anti-reflux surgery can significantly improve the patient＇ s symptoms of CVI. A permissive role of NIVCS in the pathogenicity of CVI was proposed.
