咪达唑仑粘滞型喷鼻剂治疗小儿热性惊厥疗效观察及护理 被引量:4

摘要 目的探讨自制的咪达唑仑喷鼻剂治疗小儿热性惊厥的临床疗效及实施护理干预的效果。方法将2009年1月~2011年1月来我院急诊科就诊的热性惊厥患儿120例按查随机数字表法分为对照组和观察组各60例,对照组采用咪达唑仑持续静点并辅以常规抗惊厥护理;观察组采用自制的咪达唑仑粘滞型喷鼻剂喷鼻同时给予系统的护理干预。结果两组抗惊厥均有效(P>0.05),但观察组开始用药时间和惊厥控制时间分别为(36.8±6.3)s和(188.56±53.25)s,对照组分别为(59.3±6.9)s和(259±56.03)s,观察组2项用时均明显短于对照组(P<0.01);两组家属对小儿热性惊厥知识的认知情况平均评分分别为(4.53±0.79)和(2.76±1.08)(P<0.01),随访1年患儿热性惊厥复发率11.67%对30.00%(P<0.01)。结论自制的咪达唑仑粘滞型喷鼻剂治疗小儿热性惊厥更迅速、安全、有效;有针对性的护理干预可提高患儿家属对热性惊厥的认知度,有效降低患儿热性惊厥复发率。 Objective To explore the effects of nursing intervention for Children with febrile convulsions treated with self-made midazolam(MDZ) nasal spray.Methods 120 children with febrile convulsions in emergency department of our hospital from January 2009 to January 2011 were randomly assigned into the control group and the observation group with 60 cases in each group.The control group was cared with MDZ continuous intravenous infusion supplemented by routine nursing care;and the observation group with self-made MDZ Nasal Spray supplemented by systemic nursing intervention.Results The treatments for two groups were equally effective for anti-Febrile Convulsions(P > 0.05),but the time of observation group was shorter than the control group on treatment starting and convulsions controlled(P < 0.01),the score of the parental knowledge about anti-convulsions was better too(P < 0.01).The convulsions recurrence rate was obviously reduced than that of the control group after a year.Conclusion The treatment with self-made MDZ nasal spray for children with febrile convulsions was more rapid,more safety and more effective;the targeted nursing intervention could improve the knowledge on anti-convulsions and effectively reduce the recurrence rate of febrile convulsions.
