Thrombolysis via popliteal venous catheter guided by ultrasound for lower limb deep venous thrombosis

摘要 目的探讨超声引导下经胭静脉置管溶栓治疗下肢深静脉血栓(DVT)的临床应用价值。方法回顾性分析109例髂股静脉血栓患者的临床资料,所有患者均采用超声引导下经患肢胭静脉置入溶栓导管,经导管微泵持续推注溶栓药物,同时进行抗凝治疗。结果109例患者均置管成功,其中治愈66例,显效32例,有效11例,总有效率为100%。结论超声引导下经膪静脉置管溶栓治疗下肢深静脉血栓治愈率、显效率高,缩短治疗时间,减少尿激酶用量,并发症少,值得临床推广应用。 Objective To investigate the clinical value of thrombolysis via popliteal venous cath- eterguided by ultrasound on lower limb deep venous thrombosis (DVT). Methods From April 2007 to October 2011, the data of 109 cases of lower limb deep venous thrombosis in our hospital were retrospec- tively analyzed. All the cases were performed with thrombolysis by popliteal venous catheter guided by ul- trasound combined with continuous injection of the thrombolytic drugs through catheter micro-pump, and anticoagulant therapy were undergone. Results Thrombolysis catheter was successfully inserted in all cases, 66 cases were cured, markedly effective in 32 cases, effective in 11 cases, and total technical success rate was 100%. Conclusions Thrombolysis via popliteal venous catheter guided by ultrasound for lower limb deep venous thrombosis is effective with higher cure rate, shorter treatment time, lower a- mount of urokinase, less complications, and is worthy of clinical application.
