数字化临床路径实施效果及医护人员满意度评价

Effectiveness and User's Satisfaction with Electronic Medical Record-based Digital Clinical Pathways

为评价面向电子病历应用的临床路径实施的效果及医护人员和患者或家属满意度。本文选择腹股沟斜疝、老年性白内障、不稳定心绞痛、陈旧性半月板损伤4种疾病,随机抽取临床路径实施前(2006年1月至2008年12月)和实施后(2009年1月至2011年5月)各20例,比较实施前后的平均住院日、术前住院日、平均住院费用、患者或家属满意度;并调查医护人员满意度。经比较,基于电子病历的数字化临床路径,可缩短部分手术性治疗疾病的住院时间、降低住院费用,并得到医护人员的认可。

To evaluate the effects of electronic medical record - based digital clinical pathways on quality of care and assess its acceptability by medical staff. The mean length of hospital stay and preoperative hospital stay, mean in - hospital medical cost and patient or their family, satisfaction with medical care were compared between patients hospitalized before （ pre - implementation phase, from January 2006 to December 2008 ） and after （ post - implementation phase, from January 2009 to May 2011 ） the implementation of the clinical pathways, by randomly selecting 20 cases for each of the following diseases ： inguinal hernia, age - related cataract, unstable angina pectoris, and old meniscus tear. Physician and nurse＇s satisfaction with the pathways were surveyed with a standardized questionnaire. With the result of comparison, Implementation of the electronic medical records - based digital clinical pathways may reduce the length of hospital stay and medical care cost in some surgically treated diseases. Medical staffs are generally satisfied with implementation of these digital pathways.
