摘要
目的探讨非侵袭型真菌性鼻-鼻窦炎的诊疗要点。方法回顾性分析47例非侵袭型真菌性鼻-鼻窦炎患者经鼻内镜手术的治疗结果。结果按海口标准,随访6个月～5年。其中,45例治愈;1例短期内复发,经鼻内镜换药2次治愈;1例6个月后复发,可能与真菌残留有关,经鼻内镜再次手术,未见复发。结论通过微创外科手术去除鼻窦的真菌组织,重建鼻腔鼻窦通风引流系统是手术治疗的关键。局部皮质类固醇激素的应用,可改善鼻腔黏膜的特应性状态,有效防止变应性真菌性鼻-鼻窦炎的复发,无需全身应用抗真菌药治疗。
Objective To discuss the key point of diagnosis and treatment of non = invasive fungal naso sinusitis with tiaso - endoscopy. Methods The clinical materials of 47 cases of such patient were analyzed retrospectively. Results There were 45 cases cured after 6 months to 5 year follow - up according to Haikou standard. There was a relapsed in short time who was changed dressing with endoscopy. And a case was re - operated one and half year after first time. Conclusion The micro - invasive operation could remove the fungus tissue in sinus and recon- struct ventilation. The local use of corticosteroid could change the sinus environment so that to prevent the relapse .
