缺铁性贫血患者红细胞形态分析

目的:探讨缺铁性贫血患者红细胞形态变化,为诊断缺铁性贫血提供诊断依据。方法:收治缺铁性贫血患者78例,进行骨髓学检查和外周血检查,同时选择同期健康体检的各项指标正常的健康人78例作为对照组,进行外周血检查。结果:骨髓情况:以中晚幼红细胞增生为主,见红细胞分裂相各期幼红细胞均可见体积偏小,胞浆少,核染色较正常为深,胞浆染色偏蓝或呈嗜多色性,边缘不规则,核小而致密。外周血红细胞形态:红细胞直径<6μm为主,红细胞内血红蛋白含量减低,中心淡染区扩大,可见环形红细胞。外周血HB、HCT、MCV、MCH、MCHC结果,与对照组比较差异有显著性(P<0.05)。结论:缺铁性贫血患者红细胞红细胞直径<6μm为主,红细胞内血红蛋白含量减低,中心淡染区扩大,可见环形红细胞,HB、HCT、MCV、MCH、MCHC参数有显著变化。

Abstract Objective ：To investigate iron defi- ciency anemia in patients with red blood cell morphological changes, the diagnosis of iron deficiency anemia diagnosis： Collected in our hospital from January 2011 to Decem- ber, the treatment of 78 cases in patients with iron deficiency anemia, bone marrow examination and peripheral blood examination ,78 cases choose during the same period the index of the hospital healthy normal healthy individuals as control group, periph- eral blood examination. Results：The bone marrow： in the late erythroblast proliferation mainly see red blood cell division phase of erythroblastic are visible small volume, the cytoplasm, nuclear chromatin than normal deep bluish cytoplasm or was addicted to multi - color irregular margins, nuclear small and dense. The morphology of peripheral blood red blood cells：red blood cell di ameter of 〈 6 μm - based red blood cell hemoglobin content to reduce the center lightly stained area expansion, visible ring of red blood cells. Peripheral blood of HB, HCT, MCV, MCH, MCHC results compared with the control group, significant differences （ P 〈 0.05 ）. Conclusion ： Patients with iron de- ficiency anemia in red blood cell red blood cell diameter 〈 6μm mainly to reduce red blood cell hemoglobin content, the center lightly stained area exparsion, showing circular red blood cells, HB, HCT, MCV, MCH, MCHC parameters change significantly.
