摘要 目的检测慢性阻塞性肺疾病急性发作期(AECOPD)患者血浆中血管加压素(VP)、尿液中水通道蛋白2(AQP2)水平,探讨肾脏AQP2与AECOPD外周水肿的关系。方法以ELISA法检测15例有周围水肿AECOPD的患者、15例无外周水肿的AECOPD患者、15名健康对照者血浆VP、尿液AQP2的浓度,并测定血钠及血气分析。结果 AECOPD并水肿患者血浆VP、尿液AQP2、二氧化碳分压较对照组及无水肿患者高(P<0.05),血钠较正常对照组及无水肿组低(P<0.05)。结论肾脏AQP2在AECOPD患者外周水肿的形成中起重要作用。AECOPD患者过多二氧化碳潴留能增加肾脏AQP2的表达。 Objective To study the changes of plasma vasopressin (VP) and urinary aquaporin-2 (AQP2) water channel protein levels and the relation to edema in patients with acute exacerbations of chronic obstructive pulmonary disease (AE- COPD). Methods Plasma VP and urinary AQP2 concentration of 15 healthy volunteers and 30 patients with AECOPD (15 patients with peripheral edema and 15 patients without edema) was detected by ELISA. Serum sodium and blood gas anal- ysis were measured at the same time. Results The levels of plasma VP and Urinary AQP2 and arterial blood carbon diox- ide partial pressure (PaCOz) in AECOPD patients with edema was higher than healthy volunteers and patients without edema (P 〈 0.05), while serum sodium was lower (P 〈 0.05). Conclusion Renal AQP2 plays an important role in the occur- rence of peripheral edema in paitets with AECOPD.
