奥伦自理模式对腹腔镜阑尾切除术患者早期生存质量的影响研究

The effect of orem's self-care mode to early quality of life for laparoscopic appendectomy patients

目的观察奥伦自理模式对腹腔镜阑尾切除术患者术后早期生存质量的影响。方法选择本院2011年6～12月期间行腹腔镜阑尾切除术患者94例随机分为观察组和对照组各47例,观察组给予奥伦自理模式护理,对照组给予传统护理。观察患者Barthel指数及术后恢复情况。结果观察组Barthel指数术前评分为(23.29±8.71)分,术后评分为(69.73±12.29)分,对照组术前评分为(22.34±9.43)分,术后评分为(43.01±12.10)分,两组术后与术前比较差异有统计学意义(t=21.14,t=9.24,P<0.05),并且术后观察组患者自理能力明显优于对照组,差异有统计学意义(t=10.62,P<0.05);观察组术后排气时间为(20.12±6.12)h,住院时间为(3.13±0.76)d,对照组为(42.30±5.68)h、(6.72±1.68)d,两组比较差异有统计学意义(P<0.05)。结论运用奥伦自理模式护理,能明显改善患者术后早期生存质量,值得临床进一步推广运用。

Objective To observe the effect of orem＇s self-care mode to early quality of life for laparoscopic appendectomy patients. Methods Selected 94 cases of laparoscopic appendectomy patients in our hospital, from June 2011 to Decem- ber2011. All patients were randomly divided into two groups. The observation group had 47 cases which applied orem^s self-care model nursing. The control group had 47 cases which applied traditional nursing. Observed the barthel index and the recovery after the operation of the patients. Results The barthel index of the observation group befor the operation was （23.29＋8.71） points, and （69.73＋12.29） points after the operation. The barthel index of the observation group befor the op- eration was （22.34_＋9.43） points, and （43.01_＋12.10） points after the operation. It was significantly different in the two groups before and after the operation （t = 21.14,t -- 9.24,P 〈 0.05）. And the self-care ability of the observation group was significantly better than the control group after the operation. It was significantly different （t = 10.62, P 〈 0.05）. Postopera- tive exhaust time of the observation group was （20.12＋6.12） h, and the hopital stay was （3.13＋0.76） d. They wee （42.30＋ 5.68） h, （6.72＋1.68） d in the control group respectively. It was significantly different in the two groups （P 〈 0.05）. Con- clusion Using orem＇s self-care model nursing can improve the early quality of life after the operation for the patients obvi- ously. It＇s worthy of further clinical extension and application.
