鼻假体联合带骨膜耳廓软骨矫治鼻亚单位凹陷

Nose prosthesis connect auricular cartilage with periost for treating the umbilication of nose subunit

目的:探讨采用鼻假体联合带骨膜耳廓软骨矫治鼻亚单位凹陷的方法及美学效果。方法:根据鼻部凹陷的部位和范围,按鼻美学亚单位分区设计,切取合适的不同部位、相应形状和大小的带骨膜的耳廓软骨联合鼻假体置入相应凹陷区域进行矫治。对鼻根区、鼻背区凹陷可单纯应用鼻假体矫治;对合并鼻尖区凹陷应用鼻假体联合类似"盾牌"的耳甲腔软骨矫治;对合并鼻翼区凹陷应用鼻假体联合"帽状"耳甲腔或耳甲艇软骨矫治;对合并鼻背旁凹陷运用鼻假体联合"飞燕"状耳甲腔和耳甲艇结合软骨矫治;对合并鼻小柱区短缩运用鼻假体联合耳屏软骨支撑矫治。结果:自2009年4月～2011年12月采用上述方法临床应用24例,手术切口均I期愈合。经随访3～24个月,未出现鼻假体外露或偏斜,2例仍有轻度凹陷,其余病例畸形矫正满意,外形美观,供区耳廓无明显变形。结论:应用鼻假体联合带骨膜耳廓软骨矫治鼻亚单位凹陷畸形,位置精确,软骨易于成活,吸收率少,鼻形美观。

Objective To investigate the method and aesthetics effect of nose Prosthesis connect auricular cartilage with periost for treating the umbilication of nose subunit. Methods The nose umbilication were treated by nose prosthesis connecting proper site,shape and size auricular cartilages with periost,the cartilages were obtained according to the nose subunit aesthetics and the location and extent of nose umbilication, nose prosthesis was simply used in the umbilication on the root and back of nose,the nose prosthesis loaded with cavitas conchae cartilages similar to shield was applied to the tip umbilication of nose.In the same way that the cavitas conchae or cymba auriculae cartilages similar to cap was for the wing umbilication of nose,the cavitas conchae or cymba auriculae cartilages similar to swallow was for the lateral umbilication of nose back and the antilobium cartilages was for the crispation of columella nasi correspondingly. Results 24 cases were operated from April 2009 to December 2011,all the incisions healed primarily, no prothesis reveal and deflexion,the satisfactory aesthetic results were obtained in all patients expect two cases who had slight umbilication still in the three months to two year followed up. Conclusion This is an effective operation with good nose appearance,cartilage survival easily and lower absorbance.
