摘要
目的探讨阿魏酸钠对创伤失血性休克复苏胃黏膜损伤的保护作用及机制。方法家兔100只,按随机数字表法分为4组:A组为正常对照组、B组为模型组、c组和D组分别为复苏前及复苏后应用阿魏酸钠组,每组25只。以股骨粉碎性骨折及放血法建立家兔创伤失血性休克复苏胃黏膜损伤模型,复苏前20rainC绀及复苏30min时D组缓慢静注阿魏酸钠30mg/kg。复苏90rain取胃黏膜组织,光镜下观察胃黏膜病理学改变,测定胃黏膜组织-氧化氮(NO)、内皮素(ET)和丙二醛(MDA)含量及超氧化物歧化酶(SOD)活性。结果与A组比较,余3组MDA、ET和NO含量升高及SOD活性降低(P〈0.01),胃黏膜有不同程度损伤。与B组比较,C、13两组MDA、ET和NO含量降低,而SOD活性升高(P〈0.05或0.01),胃黏膜损伤程度减轻。与C组比较,D组MDA、ET和NO含量升高及SOD活性降低(P〈0.05)。结论阿魏酸钠对家兔创伤失血性休克复苏胃黏膜损伤具有保护作用,且复苏前应用效果较佳。其机制与阿魏酸钠清除氧自由基、降低ET水平及抑制NO过量产生有关。
Objective To study the protective effects of sodium ferulate on gastric mucosal injury after traumatic hemorrhagic shock resuscitation in rabbits. Methods One hundred healthy rabbits were randomly divided into 4 groups of 25 rabbits: control(A), model(B), pro-resuscitation (C) and post -resuscitation(D) injected sodium ferulate group. The traumatic hemorrhagic shock resuscitation - gastric mueosal injury model was established by femur comminuted fracture and depletion method . In group C and D, sodium ferulate 30 mg/kg was injected intravenously 20 min before resuscitation and 30 min after resuscitation. The gastric mucosa was obtained in 90 min after resuscitation. The gastric mucosal pathological changes were examined with light microscope and the activity of SOD, content of MDA, ET and NO in gastric tissue homogenate were determined. Results Compared with group A, the contents of MDA, ETand NO were increased significantly, while the activities of SOD were decreased significantly( P 〈 0. O1 ) , and the gastric mucosal pathology changes were more serious in the other 3 groups. Compared with groupB, the contents of MDA, ETand NO were decreased significantly, while the activities of SOD were increased significantly ( P 〈 0.05 or 0.01 ), and the gastric mucosal pathology changes were relieved significantly in group C and group D. Compared with group C, the contents of MDA, ET and NO were increased significantly, while the activities of SOD were decreased significantly in group D( P 〈 0.05). Conclusion Sodium ferulate has the protective function to gastric mucosal injury after traumatic hemorrhagic shock resuscitation in rabbits, and the remedial effects was betterbefore resuscitation than ~ter resuscitation. The possible mechanism is related to the effects of scavenging oxygen free radicals, decrease the level of ET and inhibiting the excessive production of NO.
