摘要
目的探讨颈一脑动脉脉搏波传导速度(carotid.cerebral pulse wave velocity,cePWV)的测量方法及其与传统测量方法肱踝脉搏波传导速度(brachial.ankle pulse wave velocity,baPWV)的相关性。方法136名健康志愿者按年龄分为青年组(20-39岁)、中年组(40-59岁)和老年组(≥印岁),在检测baPWV的同时,采用经颅多普勒超声同步监测同侧颈总动脉和颈内动脉终末段,测量两者心动周期起跳点间的时间差以及两探头间的皮表距离,计算ccPWV。结果老年组收缩压、脉压和平均动脉压显著高于中年组和青年组。青年、中年和老年组ccPWV分别为(418±52)cm/s、(489±54)cm/s和(599±58)cm/s,老年组显著快于中年组(t=7.308,P〈0.001),中年组又显著快于青年组(t=6.758,P〈0.001)。Pearson相关分析显示,coPWv与年龄(r=0.847,P〈0.001)和baPWV(r=O.548,P〈0.001)呈显著性正相关。多重线性回归分析显示,ccPWV与年龄和舒张压均呈显著性正相关(偏相关系数分别为0.742和0.293,P分别〈0.001和〈0.010)。结论cePWV是一种新型脑血管僵硬度测量方法,与传统测量方法具有良好的相关性。
Objective To investigate the correlation between the measurement methods of carotid- cerebral pulse wave velocity (ccPWV) and the traditional method of brachial-anlde pulse wave velocity CoaPWV). Methods A total of 136 healthy volunteers were divided into a youth group (20-39 years), a middle- aged group (40-59 years), and an elderly group (more than 60 years) according to their ages. While detecting haPWV, transeranial Doppler ultrasound was used to sinmltaneously monitor the ipsilateral cotrmaon carotid artery and the terminal segment of internal carotid artery. The time differences of the beating points of their cardiac cycles and the distances of the skin surfaces between the 2 probes were measured and ccPWV was calculated. Results The systolic blood pressure, pulse pressure and mean arterial pressure in the elderly group were si^aificantly higher than those in the middle-aged group and the young group. The ccPWVs in the youth, middle-aged and elderly groups were 418 ±52 era/s, 489 ± 54 cm/s, and 599 ±58 cm/s, respectively. The elderly group was significantly faster than the middle-aged group (t = 7. 308, P 〈0. 001), and the middle-aged group was significantly faster than the youth group (t = 6. 758, P 〈 0. 001). A Pearson correlation analysis showed that cePWV was significantly positively associated with the age (r = 0. 847, P 〈 0. 001 ) and baPWV (r =0. 548, P 〈0. 001). The multiple linear regression analysis showed that ccPWV was significantly positively associated with the age and diastolic blood pressure (partial correlation coefficients were 0. 742 and 0. 293 respectively, P 〈 0. 001 and 〈 0. 010 respectively). Conclusions ccPWV is a new measurement method for eerebrovaascular stiffness, and it has a good correlation with the traditional measurement method.
