内镜多环结扎器在老年人食管静脉曲张治疗中的应用

Clinical usage of multiple band ligator (MBL) installed on endoscope for treating esophageal varices in the elderly

目的 运用内镜多环结扎器 ( MBL)治疗老年人食管静脉曲张及活动性出血的急诊止血。 方法 在内镜上安装美国 Speedband多环结扎器送达结扎部位 ,先远端后近端 ,随退镜逐层连续结扎曲张静脉。 结果 2 3例老年食管静脉曲张患者接受 MBL治疗 1～ 3次不等 ,治疗后食管静脉曲张被根治或缩小至 度 2 2例 ,治疗有效率 95.6%。其中 5例活动性出血患者经 MBL治疗均出血停止。未见严重并发症。 结论 对老年食管静脉曲张患者采用内镜 MBL治疗 ,疗效好 ,操作简便、安全。

Objective To use multiple band ligator (MBL) installed on endoscope to treat esophageal varices and to stop the hemorrhage in elderly patients. Methods Install a Speedband MBL made in U.S.A on an endoscope, send it to the varix to be ligated, then ligate one by one from distal to proximal part while drawing the endoscope out. Results Twenty three elderly patients with esophageal varices accepted the treatment of MBL and in 22 patients the varices were cured. The esophageal venous hemorrhoids were reduced to grade Ⅰ. The effective rate was 95 6%, In 5 patients with hemorrhage after ligation the hemorrhage was stopped. No serious complication was found. Conclusions Using MBL to treat elderly patients with esophageal varices is simple, safe and effective.
