摘要 目的探讨骨巨细胞瘤中CD147和Survivin的表达与其临床及生物学行为的关系。方法回顾分析我院骨病科2000年1月至2009年3月接受局部刮除手术治疗的34例骨巨细胞瘤患者的临床特点及变化;运用免疫组织化学S-P法,检测CD147和Su r,ivin在34例骨巨细胞瘤中的表达,分析其与肿瘤临床病理资料及Jaffe分级、Campanacci's分级及预后的关系。结果 CD147和Survivin在34例骨巨细胞瘤中的阳性表达率分别为55.88%和58.82%,CD147和Survivin的阳性表达率与患者性别、年龄、发病部位、Jaffe分级及Campanicci's分级无相关性,与患者的预后呈正相关。CD147和Survivin在骨巨细胞瘤中的表达强度与预后之间均存在正相关性(r=0.489,P<0.05;r=0.480,P<0.05)。CD147和Survivin在骨巨细胞瘤中的表达具有相关性(r=0.503,P<0.05)。CD147和Survivin在骨巨细胞瘤复发组的双阳性表达率(80%)明显高于无复发组的双阳性表达率(20.83%),差异有显著性(P<0.05)。结论 CD147和Survivin与骨巨细胞瘤恶性程度、发生及发展具有相关性,CD147和Survivin可作为评价骨巨细胞瘤恶性程度和判断预后的重要指标。 Objective To investigate the relationship between the expressions and the clinical and biological behavior of CD 147 and Survivin in giant cell tumor of bone. Methods The clinical features and changes of 34 patients with giant cell tumor of bone who were surgically treated by local excision in the orthopeadic department of our hospital from January 2000 to March 2009 were retrospectively analyzed. The expressions of CD147 and Survivin in 34 patients with giant cell tumor of bone were examined by using S-P immunohistochemistry, and their relationship with the tumor clinical pathology information, Jaffe grading, Campanacci's grading and the prognosis was analyzed. Results The positive expression rates of CD147 and Survivin in 34 patients with giant cell tumor of bone were 55.88% and 58.82% respectively. There was no relevance between the positive expression rates of CD147 and Survivin and gender, age, invasive location, Jaffe grading and Campanicci's grading, but there was positive correlation between the positive expression rates and prognosis of the patients. There was positive correlation between expressive intensity of both CD147 and Survivin in giant cell minor of bone and prognosis (r=0.489, P〈0.05; r=0.480, P〈0.05). There was correlation between the expressions of CD147 and Survivin in giant cell tumor of bone (r=0.503, P〈0.05). The double positive expression rates of CD 147 and Survivin in giant cell tumor of bone were significantly higher in the recurrence group (80%) than that in the group without recurrence (20.83%), and the difference was significant (P〈0.05). Conclusions CD147 and Survivin in giant cell tumor of bone are correlated with the tumor's degree of malignancy, occurrence and development, and CD147 and Survivin can be important indicators in evaluating the degree of malignancy and prognosis of giant cell tumor of bone.
