经尿道选择性绿激光汽化术与电汽化切除术治疗前列腺增生

摘要 目的比较经尿道选择性绿激光汽化术(PVP)与经尿道电汽化切除术(TUVP)对良性前列腺增生(BPH)的治疗效果。方法 570例BPH患者分为2组,PVP223例,TUVP347例。切除方法类似。对手术时间、留置尿管时间及手术前后均行国际前列腺症状评分(IPSS)、生活质量评分(QOL)、最大尿流率测定(Qmax)、残余尿测定(PVR)及并发症作比较。结果二组患者术后IPSS评分、生活质量评分、最大尿流率、残余尿均较术前低(P<0.01);二组间客观症状的改善情况差异无显著性(P>0.05);TUVP组术后继发性出血、尿潴留及电切综合征的发病率明显高于PVP组。结论 TUVP与PVP均是治疗BPH理想安全有效的手术方法,PVP手术尤其适用于不宜耐受TUVP的高龄高危患者。
