真菌作为消化性溃疡的内痈“毒热”因子的检测分析

Detection of Fungi as the Factor of Toxin-Heat in Gastrointestinal Ulcer in Gastric Membrane

目的:通过检测消化性溃疡患者胃黏膜寄居的真菌,了解作为"毒热"因子的胃黏膜真菌的菌种多样性,及与消化性溃疡的关系。方法:采集消化科就诊患者胃镜钳取的胃黏膜标本,共63人,用念珠菌显色培养基(CHROM agar)进行真菌分离培养鉴定。用玉米TWEEN80培养基进行真菌孢子形态学检查,用ITS(internaltranscribed spacer region)序列限制性酶切多态性方法(RFLP,Restriction fragment length polymorphism),检测分析真菌菌种多样性。结果:分离培养真菌32(32/63,50.8%)株,经ITS序列RFLP法鉴定白色念珠菌31株,光滑念珠菌1株。真菌阳性率与病理诊断成正相关(r=0.263,P=0.027),与性别、年龄、吸烟、饮酒、学历的相关性,均无统计学意义(P>0.05)。结论:胃黏膜寄居的真菌存在多样性,且真菌阳性率与病理损害程度存在相关性。

Objective：The fungi diversity in the gastric membrane from patients with gastrointestinal ulcer and relation- ship between the fungi diversity and gastrointestinal ulcer were studied. Methods：63 samples of the gastric membrane of patients with gastrointestinal ulcer were collected, while recording their age, gender, educational attainment, smoke, drink, denture. PCR method was preformed for identification Candida albicans and analyzing its epidemiology factors. Results： 59 samples （59/228,25.9%）, 30 families （30/58,51.7% ）were found to be Candida albicans carriers. There were 12 （12/58,20.7%） families in which only one member was Candida albicans carriers, and 18 （18/58,31. % ）families in which more than two members were Candida albicans carriers, the difference between them were statistically significant. Family had effect on Candida albicans transmission, the Kendall＇s correlation coefficient of father and son/daughter was highest（ r = 0. 587, P 〈 0. 001 ）, mother and baby was the second （ r = 0. 536, P 〈 0. 001 ）, spouse was the lowest （ r = 0. 365, P = 0. 009 ）. Candida albicans carriage was positively correlated with age （ r = 0. 139, P = 0.011 ）. The correlation between Candida albicans carriage and gender, educational attainment, smoke, drink, denture was not statistically signifi- cant（ P 〉 0. 05 ）. Concluslon：Oral Candida albicans could commensally and intrafamilially transmit, and its prevalence is related with age.
