摘要
目的:评价血管内介入联合中医辨证治疗对急性下肢深静脉血栓形成(lower extremity deep venousthrombosis,LEDVT)近期疗效的影响。方法:将2000~2011年在广州中医药大学第一附属医院就诊的急性LED-VT患者随机分为试验组与对照组,所有患者术前行下肢静脉DSA造影。试验组采用介入综合并中医辨证内外合治治疗,对照组采用介入综合治疗。随访1年以上,术后1个月、6个月根据临床疗效和影像评分标准观察记录。结果:102例急性LEDVT,试验组53例,临床总有效率98.11%,治愈率92.45%;对照组49例,临床总有效率93.88%,治愈率61.22%。两组术后6个月、试验组术后1个月和6个月临床疗效比较差异有显著意义(P<0.05)。两组介入治疗后血管通畅度比较差异有显著意义(P<0.05)。结论:介入并中医辨证序贯综合疗法对急性LEDVT的治疗安全、有效,可快速缓解临床症状,预防肺动脉栓塞,防止血栓形成复发。
Objective:To evaluate the clinical effects of the syndrome differentiation of traditional Chinese medicine (TCM) combined with interventional treatment on acute lower extremity deep vein thrombosis(LEDVT) and evaluate its feasibility and application. Methods: From 2000 to 2011, cases with acute symptomatic LEDVT determined by contrast venography and treated in the First Affiliated Hospital of Guangzhou University of Traditional Chinese Medicine were stud- ied and divided into two groups, randomly. Protocols included syndrome differentiation of TCM combined interventional treatment( experimental group) and interventional treatment( control group) beyond one - year follow - up. Results : 102 ea- ses were studied. There were 53 and 49 cases,98.11% and 93.88% of clinical efficacy rates,92.45% and 61.22% of clinical curative rates in experimental group and control group, respectively. There were significant differences in statistics on the clinical effect and venous lumen size compared with control group during 1 - month and 6 - month follow - up. Conclusion : Treatment for acute LEDVT with integration of interventional radiology and syndrome differentiation of TCM is safe and effective. It can relieve swelling and tender of lower extremity rapidly. At the same time it can prevent and treat pulmonary embolism and post- thrombotic syndrome.
