经纤维支气管镜治疗难治性肺炎84例效果分析

Treating bronchial lavage with fiberoptic bronchoscopy:Efficacy observation in 84 cases

目的:探讨经纤维支气管镜支气管肺泡灌洗治疗难治性肺炎的临床价值。方法:将84例难治性肺炎患者随机分为对照组和观察组,对照组给予抗生素与传统治疗,观察组在其基础上经纤维支气管镜吸痰与支气管肺泡灌洗术治疗。结果:观察组与对照组分别治疗1周后体温、白细胞计数及C反应蛋白有显著差异(P<0.05);观察组治疗前后血气分析各主要指标有显著差异(P<0.05),对照组除呼吸频率及动脉血氧分压外,其他血气分析指标治疗前后结果无明显差异(P>0.05);比较痰培养阳性率与平均住院天数,两组之间差异显著(P<0.05)。结论:经纤维支气管镜灌洗吸痰能提高难治性肺炎治愈率,增大痰培养阳性率,缩短住院时间,疗效显著,具有良好的临床价值,值得推广。

Objective： To investigate the efficiency of treating bronchial lavage with fiberoptic bronchoscopy. Methods： Totally 84 patients with pulmonary fungal infection were randomly divided into two groups. The treatment group （42 patients） was treated with mucus extraction combined with bron- choalveolar lavage with the fiberoptic bronchoscope, while the control group was given conventional anti bacteria treatment. Results：After 1 week of treatment, significant differences in body temperature, white blood cell count and C reactive protein were observed （P^0.05）. For the treatment group, blood gas a- nalysis indexes before and after treatment were significantly differed （P^0. 05）, but for the control group, significant difference only existed in breathing rate and PaO2 arterial partial pressure of oxygen. Significant difference in length of hospital stay and positive rate of sputum culture were observed （P〈0.05）. Gonclusion：Bronehoalveolar lavage with the fiberoptic bronehoscope can raise the effective rate of treatment, and increase the accuracy of sputum culture and shorten length of hospital stay. Thus it＇s worthy of application in large scale.
