上下颌后磨牙根管治疗失败原因回顾分析

Retrospective analysis on the reasons of the failure for treating the root canal of the maxillary and mandibular molar

目的分析探讨上下颌后磨牙根管治疗失败原因。方法对第一、第二后磨牙初次根管治疗失败病例80例进行了临床资料回顾分析。结果在80例根管治疗失败病例中,根尖渗漏29例,根管欠充填23例,根管遗漏9例,根管超充填7例,其他原因12例。结论后磨牙因其根管存在融合、变异、根管过弯曲以及对牙体解剖不熟悉,根管预备技术掌握不好等因素,导致初次根管治疗失败。掌握好根管预备技术,拍摄正位、偏位X线片,应用根管显微镜及超声技术等,进行根管治疗能获得满意的疗效。

Objective To summarize the reasons of treatment failure for root canal in the maxillary and mandibular molar.Methods Clinical data of 80 cases of first and second molar root canal who failed in first time were retrospectively analyzed.Results Of 80 cases,the apical leakage happened in 29 cases,filling shortage to root canal in 23 cases,root canal omission in 9 cases,root canal super filling in 7 cases and other reasons in 12 cases.Conclusion The treatment failure of molar root canal in first time can be caused by root canal fusion and mutation,root canal bend and surgeon’s not familiar with dental anatomy,and not handling the root canal preparation techniques.To grasp the root canal preparation techniques,shoot orthotopic and partial X-ray films,apply the root canal microscopy and ultrasound technology,root canal treatment can achieve a satisfactory effect.
