床旁超声在急危重症中的临床价值

Clinical value of bedside-ultrasound in acute and critical illness

目的探讨床旁超声检查在急危重症诊断中的临床应用价值。方法回顾性分析我院近2年行床旁超声检查的496例患者资料,其中心血管急危重患者172例,腹部急危重患者324例。结果超声诊断与临床诊断相符470例,其中急性心肌梗死76例、主动脉夹层7例、肺栓塞5例、心包积液82例、内脏损伤162例、急性胰腺炎30例、异位妊娠54例、黄体破裂38例及前置胎盘16例,符合率94.8%。漏诊26例,占5.2%。结论床旁超声检查为急危重症患者迅速、方便地提供有价值的诊断信息,具有重要的临床应用价值。

Objective To explore the clinical application value of bedeside-ultrasound in acute and critical illness. Methods A retrospective analysis was made in 496 cases which received the bedside- ultrasound examination, including 172 patients with cardiovascular acute and critical illness and 324 patients with abdominal acute and critical illness. Results In 470 of 496 cases,the ultrasonic diagnosis were consistent with clinical diagnosis, including 76 cases of acute myocardial infarction, 7 cases of aortic dissection, 5 cases of pulmonary embolism, 82 cases of pericardial effusion, 162 cases of visceral injury, 30 cases of acute pancreatitis, 54 cases of ectopic pregnancy, 38 cases of ruptured corpus luteum, and 16 cases of placenta previa. The compliance rate was 94.8%. 26 cases were missed, accounting for 5.2% among all the patients. Conclusion Bedside-ultrasound can provide valuable information quickly and easily for the diagnosis of acute and critical patients, and it has important clinical application value.
