可扩张套管下椎弓根内固定后路椎体间融合术治疗腰椎退变性疾病

Treatment for Lumbar Degenerative Diseases with Posterior Lumbar [nterbody Fusion Cage and Pedi- cle Screw Fixation under MAST QUADRANT Expansive Pipe

摘要 目的:评价可扩张套管下椎弓根内固定后路椎体间融合术(PLIF)治疗腰椎退变性疾病的临床疗效。方法:前瞻性对比研究2010年9月至2011年10月采用可扩张套管下椎弓根内固定PLIF技术(微创组)和传统PLIF手术(传统组)治疗的腰椎退变性疾病患者(分别为12例和22例)的手术时间、出血量、术后引流量、术前、术后ODl评分和椎间融合率,并对两组手术疗效进行评价。结果:微创组与传统组比较,术中出血量、术后引流量均明显减少(P〈0.05);两组间椎间融合率无明显差异,临床疗效满意。微创组术后康复时间明显短于传统组(p〈0.05)。结论:可扩张套管下椎弓根内固定后路椎体间融合术治疗腰椎退行性疾病,具有切口小、出血少、软组织损伤轻以及术后恢复快等优点,可取得与传统开放手术同样的疗效,值得临床推广应用。 Objective:To explore the early curative effects of treatment for lumbar degenerative diseases with posterior lumbar interbody fusion cage and pedicle screw fixation under MAST QUADRANT expansive pipe.Methods:A prospective comparative study was conducted from Sep. 2010 to Oct. 2011, 12 cases with lumbar degenerative diseases used minimally invasive PLIF treatment (minimally invasive group), 22 cases performed traditional PLIF (traditional method group). Surgery time, intraoperative bleeding, postoperative drainage, preoperative and postoperative ODI and interbody fusion rate were recorded. Results:Comparison between the minimally invasive group and traditional method group showed intraoperative blood loss, postoperative drainage were significantly reduced. Interbody fusion rates between the two groups had significant difference. Both achieved good therapeutic effect, but the patients of minimally invasive group recovered faster. Conclusions:The treatment of lumbar degenerative diseases with posterior lumbar interbody fusion cage and pedicle screw fixation under MAST QUADRANT expansive pipe has small incision, less bleeding, a less degree of tissue damage and rapid recovery, which can obtain the same effect as traditional open surgery. Therefore, it deserves further clinical applications under the premise of the strict indications.
