摘要
探讨以无创性方法检测新生儿耳蜗电图的可行性 ,为临床应用提供参考。方法 :应用表面电极以诱发电位诊断仪对 45例正常足月新生儿进行耳蜗电图检测。结果所示 ,所有新生儿在 12 0 d Bpe SPL强度的交替极性短声刺激下 ,均可记录到综合电位和动作电位 ,其振幅较小 ,分别为 0 .15 8± 0 .0 5 7u V和 0 .5 87± 0 .138u V,但波形清晰、稳定、重复性好。在不同 PCA足月儿间以及左右耳间各主要指标比较无显著性差异 ,提示对足月儿可采用统一的正常值标准。因此提示 :应用表面电极检测新生儿耳蜗电图的方法操作简便、结果可靠 ,属非损伤性 ,可在一定程度上反映耳蜗毛细胞和听神经的功能状态 。
To explore non invasive method of electrocochleography (ECochG) in newborn infants. ECochG was detected from scalp skin of 45 normal newborn infants by surface electrodes. Results: Summating potential (SP) and action potential (AP) were always presented in all enrolled subjects when stimuli of alternating clicks at 120 dB peSPL intensity were used. The amplify of SP and AP were lower, with average of 0.158±0.057uV and 0.587±0.138uV respectively, but the waves were all clear and stable. There were no significant differences between PCA<42 wk. and PCA>42 wk. groups, and between left and right ears with respect to all values of ECochG, indicating that normal ranges for full term newborns can be established in whole neonatal period. They may reflect the function of cochlear hair cells and hearing nerves in a considerable degree. With the advantages of non invasive and easy manipulation, this method will be valuable clinically in objective assessment of peripheral hearing function in newborn or young infant.
