动态血糖监测系统在2型糖尿病患者血糖监测中的应用价值研究 被引量:1

摘要 目的研究动态血糖监测系统(CGMS)在2型糖尿病患者血糖监测中的应用价值。方法随机抽取2009年1月至2012年1月在本院内分泌科住院并确诊为2型糖尿病的51例患者为观察对象,其中男28例,女23例,年龄42~77岁,平均(49.1±5.5)岁,病程半年~20年。随机分为两组,其中动态血糖监测组26例,常规血糖监测组25例,两组均选择诺和锐胰岛素皮下注射2次/d,两组在性别、年龄、空腹血糖、餐后2h血糖、糖化血红蛋白进行比较,无统计学差异,具有可比性。经过个体血糖监测的结果再调整胰岛素治疗后15d,两组均行72h的动态血糖监测。结果治疗后动态血糖监测组患者餐后2h血糖及糖化血红蛋白都比治疗前测定值明显降低,差异有统计学意义(P<0.05)。治疗后两组的空腹血糖(FPG)比较,差异无统计学意义(P>0.05);但两组在餐后2h血糖(2HPG)及糖化血红蛋白(血HbA)进行比较,差异有统计学意义(P<0.05)。结论动态血糖监测根据个体血糖情况调整胰岛素治疗后血糖控制情况明显优于末梢血糖监测组,能在临床广泛应用并取代末梢血糖监测成为将来的发展趋势。 Objective To study the dynamic blood glucose monitoring system (CGMS) in patients with type 2 diabetes blood glucose monitoring application value. Methods A random sample of 2009 January to 2012 January in our hospital endocrine division hospital and diagnosed as type 2 diabetesS1 cases observation object, the male 28, female 23 cases, aged 42 - 77 years old, average (49. 1 ± 5.5 ) years of age, disease course during the first half of 20 years. The paitients were randomly divided into two groups, in which the dynamic blood sugar monitoring group in 26 cases, conventional glucose monitoring group 25 cases, two groups were selected by subcutaneous injection of insulin aspart two times a day, two groups in gender, age, fasting blood glucose, postprandial 2 hour blood glucose, glycosylated hemoglobin were compared, no statistical differences, in comparison with the value. After individual glucose monitoring results and adjustment of insulin thera- py after a half of month, two groups underwent 72 hour dynamic blood sugar monitoring. Results after treatment of dynamic monitoring of blood glucose in patients 2 hours postprandial blood glucose and HbA1 c levels than before treatment determination value decreased significantly, there was significant difference and statistical significance ( P 〈 0.05 ). In two groups after the treatment, fasting blood glucose ( FPG), no significant difference(P 〉 0. 05) ; but the two groups in the 2 hour postprandial blood glucose (2hPG) and glycosylated hemoglobin (blood HbA comparison), there were significant differences with statistical significance (P 〈 0. 05 ). Conclusion Dynamic blood sugar monitoring of blood glucose insulin therapy according to the individual adjustment after the control of blood sugar was significantly better than that of peripheral blood sugar monitoring group, can be widely used in clinic and replace the peripheral blood glucose monitoring has become the development trend in the future.
