舒芬太尼与芬太尼用于全麻诱导对血流动力学及BIS的影响 被引量:3

摘要 目的观察在全麻诱导中分别使用舒芬太尼及芬太尼对气管插管时血流动力学及BIS值的影响,比较二者的效果及安全性。方法 80例ASAⅠ~Ⅱ级,择期行气管插管全身麻醉的患者,随机分为舒芬太尼组(S组)和芬太尼组(F组),每组40例,分别采用舒芬太尼0.4μg/kg、芬太尼4μg/kg进行麻醉诱导。观察麻醉诱导前(T0)、诱导后(T1)、气管插管后1min(T2)、气管插管后3min(T3)的收缩压(SBP)、舒张压(DBP)、心率(HR)、脑电双频谱指数(BIS)的变化。结果两组T1与T0比较:SBP、DBP、HR均降低(P<0.05),S组T2、T3与T0比较差异无统计学意义(P>0.05),F组T2、T3与T0比较:SBP、DBP、HR均升高(P<0.05)。F组与S组比较,T1时刻SBP、DBP、HR更低,而T2、T3时刻SBP、DBP、HR更高。两组在T1、T2、T3时刻BIS值显著低于T0时刻(P<0.05),组间比较差异无统计学意义。结论应用舒芬太尼0.4μg/kg进行麻醉诱导能更有效抑制气管插管的应激反应,并维持气管插管期间血流动力学稳定。
