经皮椎体成形术在高龄胸腰椎体压缩骨折患者中的应用 被引量:6

摘要 目的探讨经皮椎体成形术(PVP)治疗高龄(≥80岁)胸腰椎体压缩骨折的疗效及安全性。方法回顾分析高龄胸腰椎椎体压缩骨折患者37例的临床资料,共59个椎体,均为骨质疏松性或转移性肿瘤脊柱压缩骨折,椎体后壁均完整。全部在DSA引导下行单双侧入路经皮椎体成形术。结果所有患者术后6 h均能自行或由他人搀扶下床,未发现严重手术并发症。椎体前缘、中部高度及Cobb角分别由术前的(20.3±2.6)mm、(19.9±2.7)mm、(19.5±3.9)°改变为术后1 d的(23.5±3.0)mm、(23.2±2.1)mm、(15.1±2.6)°,差异有统计学意义(P<0.01);术后12 h患者胸腰背痛均有缓解,术后1 d起VAS评分及止痛药使用评分均较术前降低,差异有统计学意义(P<0.01)。结论对于高龄胸腰椎体压缩骨折患者应用PVP这种微创治疗,能够经济有效和即刻安全改善骨折椎体高度,明显缓解疼痛,早期下床活动,提高高龄患者生活质量。
