梗阻性结肠肿瘤一期切除吻合治疗体会

Treatment experience of primary excision and anastomosis for colon tumor complicated with bowel obstruction

目的探讨梗阻性结肠肿瘤一期手术切除吻合的疗效。方法回顾性分析38例结肠肿瘤肠梗阻行一期切除吻合患者的临床资料。结果右半结肠肿瘤梗阻18例及左半结肠肿瘤梗阻20例行一期切除吻合，两组患者平均住院时间（32±12）d和（33±10）d；并发症率为22．2％和30．0％；病死率为0％和5．O％，两组比较差异均无统计学意义（P〉0．05）。结论对于梗阻性结肠肿瘤患者应采用积极的治疗态度，创造手术条件，选择手术时机，采用病灶切除一期吻合可以达到满意的效果。

Objective To study the effect of primary excision and anastomosis on colon tumor complicated with bowel obstruction. Methods The clinical data of 38 patients underwent emergency one-stage resection and anastomosis of acute complete obstruction for left and right colonic carcinoma were retrospectively analyzed. Results Eighteen patients with obstructive carcinoma of the right colon were operated, the mortality was 0, the rate of complication was 22. 2%, the mean hospital stay was（32 ± 12） day. Twenty patients with obstructive carcinoma of the left colon were operated, the mortality was 5.0%, the rate of complication was 30. 0%, the mean hospital stay was（33 ±10） day. There was no significant difference in mortality or the rate of complication between the two groups （P 〉 0. 05 ）. Conclusions Primary excision and anastomosis for colon tumor complicated with bowel obstruction is feasible and effective.
